I hereby declare that I have been informed that:

The controller does not make decisions by automated means or carry out profiling as referred to in Article 22(1) and (4) of the General Data Protection Regulation.

My personal data will be processed on the basis of Article 6(1)(a) of the General Data Protection Regulation, i.e. Based on the consent I have given.

The recipients of my personal data will be employees responsible for sales and marketing services.

My personal data will be stored until I withdraw my consent to process my personal data.

I have the right to request from the controller access to, rectification, erasure or restriction of processing of personal data concerning me, as well as the right to data portability.

I have the right to withdraw my consent to the processing of personal data at any time, without affecting the lawfulness of the processing carried out on the basis of consent before its withdrawal; the right to withdraw consent may be exercised by a statement to the controller sent to the e-mail address info@fenedo.pl.

I have the right to lodge a complaint with the supervisory authority in connection with the processing of my personal data.

My provision of personal data is voluntary.
